Application forms

Requests from individuals for the issuance of documents confirming legal facts are accepted upon the applicant's arrival at the state archive, sent by post, through a messenger or an authorized representative.

Private information is provided only after the applicant confirms his identity. The applicant, when requesting to provide private information about himself, must indicate his personal identification number in the application. When such a request is sent by post or courier, it must be accompanied by a certified copy of the applicant's identity document. If the applicant's representative applies for such information, he shall additionally submit documents certifying identity and representation.

Document search services provided by the State Archives are paid. The prices of the services are determined in the price list, approved by order of the Minister of Culture of the Republic of Lithuania.
